AUTHORISATION TO DEDUCT MEMBERSHIP FEES FROM PERSAL AND OTHER IMPORTANT INFORMATION
I hereby authorize the Department to deduct from my salary at the end of each month an amount of R130,00 (Hundred and Thirty Rand), this being my membership fee to the National Teachers' Union. This authority is to remain in full force until a written cancellation is submitted by me.
